1. You Call Us
Call us for an Initial (no obligation, no hassle) phone conversation. You will speak with one of our experienced Care Managers. We will spend time asking you questions, understanding your needs, and educating you about our services and fees. Once you determine you want to partner with us, we come to you…
2. We Come To You
Our goal is to make this easy for you, whether you live near or far. Every initial consultation is different based on your needs. Generally, our first consultation will include gathering more information about your loved one, and conducting an assessment of their health, safety, and well-being in their current living situation. Supporting the well-being, safety, independence of your loved one is our main goal. Learn more about a home safety assessment.
3. We Provide Options
After we have done a comprehensive assessment, we will present you and your loved one with a care plan, recommendations, and referrals.
4. You Decide
Now that you have more information, and options you can feel confident about your decisions. We are available to help you put the care plan in place. We can continue our professional relationship and help you manage the continued medical and personal care of your loved one. And you are under no obligation to implement the plan we have provided or continue with our services.
How do I know if I need a Care Manager?
You need a Care Manager if:
- You are feeling overwhelmed and stressed by caring for an aging or disabled loved one
- You do not have the skills, energy, or desire to provide the care yourself
- Your life, family and work responsibilities already occupy too much of your time and energy
- You are weighed down with all the information and resources out there and need help making sense of it all
- You find it difficult to communicate the information and options to other family members
- Caring for an aging or disabled family member is affecting your other family and personal relationships
What are the benefits of working with a Care Manager?

- Coordinated care and communications. We will help you simplify, coordinate and manage the healthcare and personal needs of your loved one. We can serve as a central point of contact with the many healthcare providers and professionals engaged the care of your relative.
